Until that time, blue copies of dog heart testing paperwork should be sent by the Cardiologist who has performed the heart test to a central repository for record keeping – location varies by breed, details are below. Cat heart testing forms should be posted to ISFM for entry onto their “HCM-free Register”. Pink copies of forms should be returned to the primary vets of the animal, and yellow copies retained by the Cardiologist for their record. White copies are given to the owner / breeder of the animal tested.
